Volunteer Usher Application

Harford Community College prohibits possession, use or distribution of illicit drugs and illegal or unauthorized use of alcohol by volunteers in the workplace. Violations may result in action by law enforcement officials, disciplinary action up to and including termination of volunteer activities and/or mandatory participation in a substance abuse rehabilitation program. By federal law, Harford Community College must be notified within five days of any volunteer’s conviction under criminal drug statutes for conduct in the workplace. I have read and understand the above statement.

Criminal Background Screening HCC requires that all volunteers undergo and successfully complete a criminal background screening and sex offender search before being approved as a volunteer for the College. I have read and understand the above conditions for serving as a volunteer. By signing this information form, I acknowledge and accept the aforementioned conditions of volunteering and grant HCC and its authorized vendor(s) permission to conduct investigative consumer reports to the extent permitted by law.

I certify the information provided on this application and any attachments, including but not limited to my resume, to be correct and complete to the best of my knowledge and belief. I make this certification to Harford Community College with knowledge that any false or misleading statement or omission of material fact may be sufficient cause for disqualification of the applicant or dismissal if employed. I authorize Harford Community College to verify any or all information I have submitted. I further release, promise to hold harmless, and covenant not to sue HCC on the basis of its attempts to obtain information from any previous and present schools and employers.
